To help how many clans are being made and killed so fast.. And to help the new people who are making clans as they start, to not rack up a huge record.
How about some options:
When you press the "leave clan" button, it can ask Why are you leaving? And have some of these reasons to pick from:
[ ] Joining a different clan
[ ] Making a new clan
[ ] Too busy for a clan right now
[ ] Quitting this game
After selecting a box, it can say something for each one.
Box 1: Reason for a new clan?
[ ] Dislike the member(s) of this one
[ ] Found a better one
[ ] Other ________
Box 2: You can PM jitspoe to change your clan name or tag if you are just going to make a new clan for the name or tag.
[ ] No, I want to make a new clan - Leave
[ ] Yes, I'll PM jitspoe to have it changed - Don't leave
Box 3: You can always be on a clan and labeled "inactive" by the other members. You can still remain in a clan while not playing. Are you sure you want to leave?
[ ] Yes, I want to leave while I'm too busy to play
[ ] No, I'll stay in the clan
Box 4: Reason for quitting?
[ ] I don't like this game / lost interest in it
[ ] Too busy and time consuming
[ ] Found another game I like better
[ ] Other ________
Some other options:
Under Box 2 -> Yes, I'll PM jitspoe to have it changed, maybe something that can automatically send a message to Jits saying the new name, tag, or both that they want changed. And a note that it will only be changed if the name / tag is available, and a limit of changes in a certain period of time.
Also this could be implemented with the "clan contracts" ideas that people have suggested.
This would probably need to have a language(s) option so more people can understand what it's saying.

It would be a great idea, but knowing what this community is currently like, a lot would just ignore it and tick any box.

The key question is: Who would bother looking at this collected data?
IMHO, if you aren't doing anything constructive with collective data like this, there is no reason to deploy a questionaire.

True knack. So maybe this should only apply for clan members. This would be emailed to the leader(s) of the clan.

You would think someone leaving a clan would tell the leader why they are leaving before they do. I think there are many more important features that need to be implemented before this. This seems like more work to do than it is worth.
